Returning Student Annual Verification Form

In an ongoing effort to keep our student information as current and accurate as possible and maintain an awareness of important district policies, the West Springfield Public School District conducts an annual verification process. Families are asked to review the information the district has on file and report any changes, as well as read and acknowledge the included policies. We encourage families to complete this process via the online form.

At the beginning of the school year paper packets with this information will go home to every student. The paper forms DO NOT need to be completed if the online form is used.

Please note that the nursing staff will still require the Pink Health History Form to be filled out by hand and returned to your student's school complete with signature. And, there may be school specific forms required as well.

The Returning Student Annual Verification Form is accessed through the Parent Portal. If you do not have a Parent Portal account, please refer to your Parent Portal Letter for detailed instructions or, if you can not locate the letter, contact or visit your school's office for help.

How do I get started?

Start by visiting the PowerSchool Parent Portal

From the Parent Portal:

  1. Select the student you wish to register along the top
  2. Select the Returning Student Annual Verification Form Icon located on the menu panel
  3. Select “New to PowerSchool Enrollment” and Agree to the terms and conditions (click to comply with COPPA)
  4. Click Begin Forms
  5. Enter your student’s Date of Birth (MM/DD/YYYY) and press continue
  6. Begin form and press SUBMIT when form is completed
  7. To update additional students, return back to Parent Portal and click on your next student and select the Returning Student Annual Verification Form Icon on the menu panel
  8. Enter your student’s Date of Birth (MM/DD/YYYY) and press continue
  9. If you would like to import the relevant information to save time, select “Import”

I can’t remember my login for the PowerSchool Parent Portal.

If you aren’t able to retrieve your login credentials using the “Having trouble signing in?” link on the Sign In page, please contact your school directly for assistance logging in to the Parent Portal.

Do I have to answer all the questions?

Required questions are marked as "Required."

What if I make a mistake?

If you would like to make a change, click on the underlined field or click “Previous” to return to a previous page.

I’ve completed the form, now what?

When you have finished entering your information, click “Submit.” This will send all of the information you’ve entered to the school. If you cannot click on this button, you will need to make sure that you have answered all required questions.

What if I have more than one student in the WSPS District?

If you have more than one student you do need to fill out the form for each one because you’ll need to provide information that is specific for each child. We recommend that you submit one Returning Student Annual Verification Form and then start another – this will allow you to “snap over” shared family information, which will save you time.
